A division of Wits Health Consortium (Pty) Ltd • Daily review of clinical records to ensure that all beneficiaries/clients identified as in need are followed up to link them to appropriate care and support services, including follow-ups to ensure clients attend their scheduled appointments. • Provide individualized support to beneficiaries/clients accessing mobile services in communities, including follow-up contact via phone calls or SMS. • Provide information to beneficiaries/clients and ensure they have a good understanding of HIV prevention and care pathways. • Ensuring linkage to care for all HIV-positive clients and same-day treatment initiation through a ‘handshake’ approach with anchor facilities. • Liaise with DOH facility staff and district support partners to overcome barriers to ART initiation pathways. • Help beneficiaries/clients make appointments for first visit or follow-up visits. • Ensure that HIV-negative beneficiaries/clients interested in PrEP have access to other prevention resources, as appropriate, prior to initiation of PrEP and at follow-up visits.
