Our organization maintains a mandatory COVID-19 requirement and as such only incumbents vaccinated against COVID-19 will be considered for positions. • Manage community profiles and map systems for linking beneficiaries/clients from the community to the establishment and vice versa. • Identify community structures to provide ongoing support to beneficiaries and facilitate linkages with beneficiaries as needed. • Build strong working relationships with the Integrated School Health Program and DREAMS school structures and implementing partners to ensure proper utilization of risk and vulnerability criteria and provide effective two-way linkages. • Link clients/beneficiaries to DREAMS services to ensure blending with other prevention, HNV, post-violence care, care and treatment services based on specific needs identified. • Facilitate linkages for beneficiaries/clients with local NGOs, FBOs, CBOs and any other relevant support systems, as appropriate. A division of Wits Health Consortium (Pty) Ltd • Daily review of clinical records to ensure that all beneficiaries/clients identified as in need are followed up to link them to appropriate care and support services, including follow-ups to ensure clients attend their scheduled appointments. • Provide individualized support to beneficiaries/clients accessing mobile services in communities, including follow-up contact via phone calls or SMS. • Provide information to beneficiaries/clients and ensure they have a good understanding of HIV prevention and care pathways. • Ensuring linkage to care for all HIV-positive clients and same-day treatment initiation through a ‘handshake’ approach with anchor facilities. • Liaise with DOH facility staff and district support partners to overcome barriers to ART initiation pathways. • Help beneficiaries/clients make appointments for first visit or follow-up visits. • Ensure that HIV-negative beneficiaries/clients interested in PrEP have access to other prevention resources, as appropriate, prior to initiation of PrEP and at follow-up visits.
